The Federal Reserve System (Fed) is the principal monetary authority or the central bank of the United States of America. In the 1890’s the United States attitude turned much aggressive and expansionistic than it had ever been. There was in fact a lust for forming colonies. What triggered the war was the blowing up of the Maine Battleship. The blame was put on Spain’s shoulders believing the Spanish had a hand in the destruction of the Maine. Years after the end of the war investigators have concluded that the Maine blew up due to the boiler room overheating and blowing up. The Spanish-American War is the most unnecessary war of all-time and could have easy been avoided if America did not declare wars on other nations solely on belief without any evidence. The defeat of the Spanish forces marked the end of their rule in the Americas and also marked the rise of the United States as a global military power. The Spanish-American War affected the United States in a number of other ways. It helped speed the construction of the Panama Canal and also resulted in the U. S. s acquisition of foreign territories. This war marks the being of American Imperialism, and the beginning of American intervention in other countries. Before the war and before the explosion of the USS Maine there was a lot of tension between America and Spain at the time. The United States and Spain all most started a war when Spain captured an American ship in 1873. The captain and the crew were executed on charges of having rebel leaders aboard (Gay 9). There were many reports of U.  S citizens in Cuba being imprisoned or murdered. Spanish officials even confiscated U. S property (Gay 9). Also, Americans living in Cuba were captured and sent without trial to this prison known as Morro Castle (Gay 23). Also, Cuba was rebelling against Spanish rule and wanted its own independence. Constant fighting between Spain and the natives became deadly. Cuba was an important colony for the Spanish. The islands export sugar, tobacco, and other raw materials were traded by the Spanish to generate great amounts of wealth (Bachrach 15). In January 25, the USS Maine arrives in Havana, Cuba. The Maine is there to protect Americans in Cuba against the riot occurring in Havana (Bowan 103). February 15, the USS Maine while docked in the Havana harbor is rocketed by two explosions. The ship begins to sink killing 266 U. S sailors (Bowan 104). The United States places the blame upon Spain for the destruction of the Maine and in April the United States and Spain break off diplomatic relations. The U. S president orders a naval blockade of Cuba’s ports (Bowan 104). McKinley stressed that the United States had tried to remain neutral, but too many Americans were treated inhumanly in Cuba by the Spanish (Gay 28). In 1823, the Monroe Doctrine stated that further efforts by European governments to colonize land or interfere with states in the Americas would not be accepted by the U. S. , but Spains colony in Cuba was exempted. In 1890, Captain Alfred Thayer Mahan wrote The Influence of Sea Power upon History, which credits the rise of Britain to world power to the Royal Navy. Mahan’s ideas on projecting strength through a strong navy had a powerful worldwide influence. Historians debate how much Americans were interested in obtaining an empire, while noting that the European powers had in recent decades dramatically expanded their empires, especially in Africa and Asia. The United States had great interest in the England motto, which was make the world England. The United States wanted to colonize, take over new territories, and expand. Unfortunately, America’s ambitions were inopportune for the current state of the world, because most of the world was colonized and had governments. America probably pinned the blame of the Maine blowing up on Spain to grab new territory and some other spoils of war. On April 23 President McKinley called for a hundred twenty five thousand volunteers, more than a million men answered the call (Golay 41). The U. S Navy’s Asiatic Squadron, is headed by Commodore George Dewy. He is based in Hong Kong, China. His job is to combat the Spanish in the Pacific (Bowman 104). Amazingly the U. S had so little information about the Philippines that Dewey had to buy charts of Manila Bay from Hong Kong (Golay 22). In the early hour of May 1, 1898, Dewy spots the Spanish fleet 5 miles south of Manila. The U. S fleet sails towards the Spanish fleet (Bowman 107). Dewey’s ships have destroyed the majority of the Spanish fleet and captured the remaining Spanish fleet. The Spanish lose more than 400 men. Only 6 Americans die or are wounded. When word reaches America of Dewey’s victory he becomes a national hero (Bowman 107). Dewy stays in Manila harbor with his fleet to keep the German Navy from the weak situation in the Philippines. The U. S does not want Germany to take territory or resources from the Philippines (Bowman 107). Following Deweys victory, Manila Bay was filled with the warships of the United Kingdom, Germany, France, and Japan; all of which outgunned Deweys force. The German fleet of eight ships were in Philippine waters to protect German interests acted provocatively cutting in front of American ships, refusing to salute the United States flag, taking soundings of the harbor, and landing supplies for the besieged Spanish. The Germans, with interests of their own, were eager to take advantage of whatever opportunities the conflict in the islands might afford. The Americans called the bluff of the Germans, threatening conflict if the aggressive activities continued, and the Germans backed down. At the time, the Germans expected the confrontation in the Philippines to end in an American defeat, with the revolutionaries capturing Manila and leaving the Philippines ripe for German picking. Most of the Spanish fleet is anchored in Santiago. The commander of the Spanish fleet is Pascual Cervera. He does not want to challenge the U. S, because he knows that he is out gunned (Bowan 109). On 1 July, a combined force of about 15,000 American troops in regular infantry and cavalry regiments, including all four of the armys Colored regiments, and volunteer regiments, among them Roosevelt and his Rough Riders, the 71st New York and 1st North Carolina, and rebel Cuban forces attacked 1,270 entrenched Spaniards in dangerous Civil War-style frontal assaults at the Battle of El Caney and Battle of San Juan Hill outside of Santiago. More than 200 U. S. oldiers were killed and close to 1,200 wounded in the fighting. Supporting fire by Gatling guns was critical to the success of the assault. Cervera decided to escape Santiago two days later. The Spanish forces at Guantanamo were so isolated by Marines and Cuban forces that they did not know that Santiago was under siege and their forces in the northern part of the province could not break through Cuban lines. This was not true of the Escario relief column from Manzanillo, which fought its way past determined Cuban resistance but arrived too late to participate in the siege. After the battles of San Juan Hill and El Caney, the American advance ground to a halt. Spanish troops successfully defended Fort Canosa, allowing them to stabilize their line and bar the entry to Santiago (Bowan 109). The Americans and Cubans forcibly began a bloody, strangling siege of the city. During the nights, Cuban troops dug successive series of trenches toward the Spanish positions. Once completed, these parapets were occupied by U. S. soldiers and a new set of excavations went forward. American troops, while suffering daily losses from Spanish fire, suffered far more casualties from heat exhaustion and mosquito-borne disease. At the western approaches to the city, Cuban general Calixto Garcia began to encroach on the city, causing much panic and fear of reprisals among the Spanish forces. The success at the two forts on July 1, 1898 combined to give the Americans command over the ridges surrounding Santiago. By July 3, the American forces had demolished Admiral Pascual’s Spanish fleet there. On July 17, the Spanish surrendered the city (Nelson 111). There was likelihood that the Spanish could carry the War onto the Atlantic seaboard cities of the U. S (Golay 31). The Spanish Admiral Cervera could not have raided the U. S coastal cities because they had no charts of the Atlantic Seas (Golay 33). The Spanish fleet attacks the seven American ships. After four hours of gunfire the Spanish ships are all sunk. The Spanish have 474 dead while the Americans only suffer 1 fatality (Bowan 109). On 7 August, the American invasion force started to leave Cuba. The problem was fiebre amarilla, yellow fever, which had quickly spread amongst the American occupation force, crippling it. A group of concerned officers of the American army chose Theodore Roosevelt to draft a request to Washington that it withdraw the Army, a request that paralleled a similar one from General Shafter, who described his force as an â€Å"army of convalescents† (Bowan 111). By the time of his letter, 75% of the force in Cuba was unfit for service. The evacuation was not total. The U. S. Army kept the black Ninth Infantry Regiment in Cuba to support the occupation. The logic was that their race and the fact that many black volunteers came from southern states would protect them; this logic led to these soldiers being nicknamed â€Å"Immunes†. Still, by the time the Ninth left, 73 of its 984 soldiers had contracted the disease. The Treaty of Paris was signed December 10, 1898, settled the conflict that had resulted in the Spanish-American War( Nelson 111). As a result of the Spanish-American war, which lasted for only four months from April 25 to August 12, 1898, the treaty gave Cuba independence. Andrew Carnegie the richest man in America wrote a personal check for twenty million dollars to buy Philippine independence (Marrin 150). The U. S would pay 20 million dollars for the Philippines. The Philippines would become a common wealth in November 1935 (Nelson 112). U. S. also acquired Puerto Rico and Guam. By gaining these territories it helped America gain some more resources. Also the result of the war leaves thousands dead on each side of the playing field and many more wounded. These deaths could have been avoided if only the United States did not jump to conclusions and did not use the Maine incident to enter a war in order in gain new territory. In the end, U. S. goals were overwhelmingly achieved. They succeeded in securing Cuban independence, removing Spanish forces from the Americas, establishing themselves as a world and military power, and also accomplished much more, with minimal losses. Other positive outcomes from the war include a positive change in the army, a surge in the economy, and as well as a strong international political influence. This eventually brings to confusion and conflicting opinions as the professional identity of nursing become unclear and the blurring of boundaries are existing inadvertently. Barber (2002) commended that the traditional professional identity of nursing might be lost in the context of the expanded role by the nurse. The nurse tends to pass the traditional nursing role to others junior staff or healthcare assistants whenever they extended their scope of practice which usually done by doctors. As many others hospital setting, the problem in my working area are lack of doctors and the nurses appear to be the largest group in the health setting. The article( Star on line) (2010) reported that in Malaysia, the doctor and patient ratio is 1:940 in year 2009 and the nurse patient ratio is 1:645 in year 2009 and 1:375 in year 2010 as reported in The Star on line (2008). Thus, In the context of lacking of doctors, the nurses tend to takeover the expended role voluntarily as they are willing to learn and developing themselves. The expanded role usually performed by the staff in my ward is cannulation, this happened whenever the doctors are busy covering a few wards during night duty. No doubt it actually greatly increased their burdens, but they do have their key role in coordinating patients care and look after them from the organizational turbulence. Rushforth Mc Donald (2004) had done a survey on the expanded role of nurses on procedure such as IV admistration, Cannulation, ECG recording, Defibrillation and Prescibing, they found that on cannulation alone are performed by over 50% of the nurses in the respective area. The female medical ward which has 35 beds is a busy ward and one of the highest admission rate in the hospital, the turnover rate is high as the number of admissions. The bed occupancy rate was xxx% and a total number of xx patients were admitted in 2009. The average length of stay was xx days, the ward has divided into five cubicles and 2 isolation rooms for the patient either receive chemotherapy and cases of Methicillin Resistant Staphylococcus aureus (MRSA) and Extended Spectrum Beta Lactamases ( ESBL). The total number of nurses in my unit are 27 and eight out of them are senior staff nurses. Most of the patient admitted is due to hypertension, diabetes mellitus and anemia and they need the cannulation for the continuation of injection antibiotic, maintenance of fluid to prevent dehydration and blood transfusion purposes. In this paper, I will discuss the crossing boundary in the context of cannulation being done by nurses. Although the senior staffs are given the authority by doctors in performing the cannulation to the patients, sometime they are facing the situation of the role uncertainty and decision making in expending their role. In the United Kingdom, the neonatal nurse are given the title of Advanced Neonatal Nurse Practitioner after completed the course, they are able to perform the duty as the junior doctors competently and efficiently in handling the cases of the high risk infant as described by Dilon George ( 1997). The diabetes specialist nurse provide the need of the patient relating to diet control, self monitoring and treatment as illustrated by Loveman et al ( 2003). Similarly in Malaysia, the diabetes nurse who are educating the diabetes patients and neonatal nurse who are taking care of the newborn infant are not given the title such as nurse practitioner, advanced nurse practitioner and clinical specialist nurse. Those nurses are undergone specialized post basic training and most of them are quite senior but always been referred as neonatal nurse, diabetes nurse, stoma nurse, critical care nurse and so on. Nancarrow (2004) illustrated that increasing in the overlapping of the extended role is result from the growth of the interprofessional practice and training. It happened among nurses and others healthcare providers such as doctors, dietician, pharmacist and physiotherapist which has brings to the establishment of the new nursing roles. However, Cameron (2010) has found that the senior nurses felt empowered in expanding their role and appear to be more acceptable about flexible work and multi-skilling to improve continuity, efficiency and effectiveness of patient care compare to the junior staffs who felt more stressful in the expanded role. I am working as a staff nurse in a medical ward and have 18 years of experience. From my experience, I noticed that the senior staff frequently making recommendations regarding patient care and they do influence decision making by providing related information about the patient. This is always happened spontaneously during the interaction in between the nurses and the doctors. In my practice setting, the ward are monopoly by doctors because of they are seen owing full professional status by having exclusive theoretical knowledge which enable them to control over nurses. However, the junior doctors are rotating every three month and they are relied on the senior nurses for guidance on the aspects of ward practice and the details of local protocols. For instance, in the treatment decision, they will seek for the nursing advice about drug dosages if they are unclear. The junior doctor is responsibilities to cover a few medical wards included acute medical ward which has a few ill and ventilated patients. The problem arise whenever the nurses facing difficulties in getting the junior doctors to come to the ward to set cannula resulting the burden has increase to ward staff. The nurse must undertake the cannulation themselves either in providing the continuity of circulatory antibiotic levels or to ensure the maintena nce of adequate fluid status for the patient. However, Tye and Ross (2000) has illustrate that an evaluation done by the key stakeholders within the organization indicates that the extended role of the nurses may culminate in uncertainty, individual variation and a perception of a blurring of role boundaries. Hoder and Schenthal (2007) described that professional boundaries are defined as limit-lines protection in between the nurse and the patient. The demand for professional boundaries has increasing drastically in nursing profession, resulting the limitation must be set to ensure a safe, therapeutic connection among the nurse-client relationship. An essential component of competent and ethical nursing care is required in providing better services and reducing underlying conflict. It is the responsibilities of the nurses to establish and sustain the professional relationship through the guidance of code of ethics and professional standards. Most of the nurses in my ward are facing with the challenges even though they are given the authority in setting the cannula, this is because they have to make difficult decisions and facing uncertainty whenever they are dealing with difficult situation. In minimizing the uncertainty, they are practicing a few routes to ensure the best services are delivered to the client. First, through the documentation of evidence based practice on client preference, the nurse must get the consent or agreement from the client before delivering the services to him/her. Besides that, the NNIA ( National Nursing International Audit) are implemented to ensure the current practice did not increase infection and insertion site phlebitis, the audit is carried out by the nursing team in my practice area every twice yearly. Secondly is the source through SOP( Standard Of Procedure) and protocol or guideline that is easily accessible for nurses as referral when need. The nurses are practicing the proper hand washing technique and performing the procedure under aseptic technique to avoid infection during performing the cannulation for client. From a professional perspective, the Code of Professional Conduct for nurses (1998) states that individual nurses have a responsibilities to keep up with advances in health practice and maintain their competence in nursing knowledge and skills. Aveyard and Sharp (2009) illustrate that by performing the latest evidence based practice is able to protect the healthcare provider from any legal proceedings. The third source is the intuitive knowledge that emerged from the past experience. Benner (1984) has described a nurse must passes through five stages that is from novice to expert. Although the contribution of knowledge has been acknowledge in clinical practice, most of the credibility is subjected to biasness. This is supported by Romyn et al (2003) said that credibility must base on knowledge that used to be critique and scrutinized. In order to perform the task of decision making, Dowding and Thompson (2003) stipulated the nurses must aware that the impact of decision making are influence on nursing outcome and patients experience. Simpson and Courtney (2002) emphasized that the nurses must be able to think critically to provide best care at the same time dealing with the expansion role especially in the current health care system as it appears to be more complex over the last few decades. As suggestion, Rushforth and McDonald (2004) recommended that a sense of balancing and safer policy is needed for the few senior nurses to expand their role without jeopardized to the nursing component. The nurses are fit to practice and accountable for the nursing care they provide. Oermann(2007) suggested that evidence based practice in nursing can be searched in the web sites. Inevitably, there is far-reaching implication pending on how the nurses conduct themselves in their relationships. If the blurring boundaries always happened, the nurses will be confused as it bring into an ethical issue regarding their professional role. Barber (2002) suggested that all practitioners are responsible toward their action and have high awareness of legal implication of practice during performing their overlapping role. However, Molyneux (2001) described that contribution of care from multidiscipline for the same patient through collaboration, knowledge and skills. Caldwell and Atwal (2003) found that different ideologies, unequal power, overlapping role and communication are the difficulties exist within the interprofessional practice. Therefore, a clear flow and structured at several level is needed for the interprofessional team to facilitate and guide their practice effectively. As conclusion, no doubt the nursing blurring boundaries may against the professional rule, but it had bring benefit to patients especially during night time and the treatment are able to carry out on time. Nurses must understand the additional responsibility invariably probably may increase their burden. Therefore, by using evidence-based practice, critical thinking and updating knowledge in lifelong learning, the nurses are able to extend and cross the boundaries safely to meet clients needs.

American society was a society of opportunity. Americans felt that, given a chance, they could make a better life for themselves. This was the era of the common people, the era of democracy. Andrew Jackson appealed to the American people because he stood for values many regarded with favor. However democratic Jackson may seem, he was more tyrant-like than any of his predecessors. His major offerings to the nation included majority rule and a popular presidency, however offered no benefits to women, African Americans, nor Native Americans. Jacksonian Democracy was in no way democratic.   Ã‚  Ã‚  Ã‚  Ã‚  Before Jackson's time, voters expected public officials to use their own best judgment in electing. Under Jacksonian Democracy, the people came to believe that officials should act according to the demands of the people. To make government respond more directly to the popular will, state and local governments began to fill some positions such as judges, constables, and public surveyors by election rather than appointment. The terms of office were also shortened so that popular opinion had a more direct effect on the actions of elected officials.
